To successfully register as player or coach, you must:

  1. Review the MYA Code of Conduct and read the CDC's Concussion Fact Sheet for Parents.
  2. Complete all required information for each player, coach or referee within your family. Please scroll down this page to find the appropriate registration form.  Multiple registrations should be generated through one family account. Please do not set up a separate account if you want be a coach or referee.
